Case for Discussion Expanded
Hazel Smith is a 3 year old white child who is being brought to an Infant Mental Health Program at the Mental Health center by her 20 year old mother Gladys Jones. Ms. Jones was referred by her CalWorks worker because of her difficulty in following through on her work/education program and her inability to hold a job. This difficulty is related to her inability to sustain Hazel in a preschool program. Hazel has been kicked out of 4 programs because of her aggressive behavior. She bites other children, hits them and takes their toys. Other parents complain about her behavior and the facilities do not feel as if they can control her.
Ms. Jones is a single mother and this is her only child. Hazel’s father is Ms. Jones high school boyfriend who is now in jail for selling drugs. Ms. Jones dropped out of high school in her junior year because of her pregnancy. Her parents helped her for a while, but are now telling Ms. Jones that she has to find her own place to live because they cannot tolerate Hazel’s behavior.
In the interview, Ms. Jones tells you that Hazel is a “bad child” and she has tried everything. You notice in the interview that Hazel does not look to her mother for comfort and is in to everything in the playroom. Ms. Jones yells at her to stop and slaps her hands. Hazel makes little response to her mother, but as you are getting a history, you notice that Hazel is hitting the doll and saying “you are a bad bad girl’.
- How does Hazel’s cognitive developmental level and socio-emotional development influence your understanding of the family?
- Are there any special tools you might need for your assessment of Hazel?
- Are there any additional domains you would assess beyond your first look at this family?
- What strengths do you see in Hazel and Ms. Jones?
- What are the problems that are most concerning and would be the focus of your attention?
- How are you thinking about intervention?
- Are there any cultural issues important in your thinking?
- Plan a role play for this scenario.
Alternative Scenarios
- Let’s change the scenario and let Ms. Jones be African-American. Does that have implications for engagement?
- Let’s again change and assume that Ms. Jones is Latina and her parents emigrated from El Salvador after the conflict there. Does that have implications for engagement?
- And now let’s assume that Ms. Jones is Korean-American and her family came here from Korea when she was in middle school. Hazel’s father is not Korean-American.
