Mr Jack Smith is a 43-year-old male, married with 2 children, who works as a surveyor for a local builder. He was involved in a MVA on an outer metropolitan road, on the way home from work. The estimated speed of the vehicle was 90km/hr when the driver veered to miss a large pothole in the road. Mr Smith was the passenger in the vehicle and his colleague, the driver of the vehicle, died at the scene.

A car travelling behind the vehicle witnessed the accident and immediately called emergency services. Mr Smith was confused and disoriented at the scene. He was unable to be easily extricated from the car due to severe pelvic pain and pain to his left arm.

He arrived in the emergency department (ED) of the nearest hospital having been brought in by ambulance 50 minutes after the accident.

Signs and symptoms

His left forearm was swollen out of alignment, vacuum immobilser insitu and elevated on a pillow. He has a large abrasion to his forehead and over his left zygoma. He has a range of small lacerations to his face, arm and chest from debris. He is complaining of generalised abdominal pain and pelvic pain. When he tries to move on the barouche, he describes his pain as unbearable. He already has the presence of ecchymosis from left shoulder to right hip. Cervical collar insitu
